from what i read, the skin and the cartilage splits (probably from impact or friction), and fluid builds up in the space between (a mixture of blood and pus). the cartilage eventually dies out, deforming the ear.
from what i learned, friction causes formation of liquid in the surface of the ear. pretty much like a blister. impact spreads the space, making the blister bigger. after a week, the blood starts to clot, hardening the deformities in the ear. you could have the fluid drained out before it hardens.

first time i've seen one that close, i've knew some people who have it but never really looked at their ears that much...
